What to keep

  1. Diagnosis: type, stage and receptor results.
  2. Results: pathology, scans and blood tests, with dates.
  3. Treatment plan: drugs, schedule and goals.
  4. Medicines and allergies: current list with doses.
  5. Contacts: nurse specialist, oncologist, out-of-hours number.
  6. Appointment notes: date, what was said, next steps.

A simple system

  • One physical folder with sections, one folder on your phone with photos or PDFs.
  • Name files with date first, for example "2026-11-03 scan report".
  • Ask for copies of results at each visit, or use your hospital portal.

Share carefully

Share records only with people you trust, and ask your team how to send them safely.
